ONLINE ENROLLMENT & REGISTRATION POLICY
- Register online at www.sbps.net/home. Please indicate previous school attended with their address and phone number listed for records requests. It is VERY important to have this information in order to see academic history and what credits you need to achieve graduation requirements PRIOR to your appointment. Please provide birth certificate and proof of Scottsbluff residence at the upload prompt provided. Option In applications can be obtained at the SBPS District Offices on 1722 1st Avenue in Scottsbluff.
- Once the Counseling Office is notified of your online enrollment, requests for records will be sent to your previous school. This may take time to receive as schools, especially those that are out of state; some to most schools may not return from summer break as soon as we do.
- Upon receipt of records and Counselor Review, our office will reach out to you for your registration appointment. Please provide proof of address at the time of your appointment, i.e., utility bill, driver's license, etc. if you did not include these on your online enrollment. This insures the most accurate household information for emergency purposes. (If you have not previously done so on the online enrollment)
- Your street address MUST be within the Scottsbluff School District or an option must be on file at the SBPS Administration Office. Attendance area map can be found under QUICK LINKS on our SBPS District Home Page.
- Options - If the student is optioning into Scottsbluff High School and lives outside of attendance area, please be aware that if an option is NOT signed, filed and approved by March 15 of the prior year, the student athlete may NOT compete at the varsity level for ninety (90) school days.
- If you are enrolling from out of the country, please bring with you any educational history, immunizations and certificate of birth and legal documentation of entry to the United States

Requirements to Enroll at Scottsbluff High School:

Identification - Certified Birth Certificate
Nebraska State Statute and Board Policy require reliable proof of the student's identity and date of birth upon enrollment for the first time in the Scottsbluff Public School District. The person enrolling the student must provide a certified copy of the student's birth certificate. (Certified shall mean a certificate with a raised seal as issued by the Bureau of Vital Statistics.)

Immunization Records
The Immunization Law in Nebraska requires schools to have a record on file of a student's immunization history before they can attend school. Nebraska Law requires that students shall be protected against poliomyelitis, Diptheria, Pertussis,Tetanus (DPT), Measles, Mumps, Rubella (MMR), Hepatitis B, and Varicella (chicken pox). Any student who does not comply with immunization requirements shall not be permitted to continue
in school until he or she shall so comply. A student may be provisionally enrolled if he or she has begun the immunizations required under Nebraska Law. For more information, please contact the school nurse in your student's building of enrollment or call 436-1655. All students enrolling at the beginner grade (kindergarten or first grade) through twelfth grade, including out of state transfers to any grade, are required to show record of: Three doses of DtaP, DTP, DT, or Td vaccine with at least one dose given no earlier than four (4) days before the fourth birthday; three doses of Polio vaccine; 2 doses of MMR vaccine with the first dose given no earlier than four (4) days before the first birthday and the two doses separated by at least 28 days; two doses of Varicella, with the first dose given no earlier than 4 days before the first birthday and each dose given at least 28 days apart OR provide a signed parent/guardian statement of past history of chicken pox disease, including year of illness; three doses of pediatric Hepatitis B vaccine, or if the alternate Hepatitis B schedule is used, two doses of adult Hepatitis B vaccine specified for adolescents 11-15 years of age. The alternate two-dose schedule must be administered with at least 112 days between #1 and #2, and must be completed before the 16th birthday.
Additional Note: One Tdap (Tetanus, diphtheria, acellular pertussis) is required for entry to 7th grade. The vaccination can be given after the seventh birthday, depending on the brand of vaccine received and prior to entry to 7th grade. Please submit a copy of your student's immunization record to the Health Office. For waiver information, please call 436-1655 or contact the school nurse at the school of enrollment. New waivers for medical or religious reasons will require a physician or Notary signature. Waiver forms are available from your school secretary or school nurse.

Physical Examination Forms
Parents are reminded that all children entering kindergarten, seventh grade or transferring from out of state must have a physical examination prior to the start of the school year. All kindergarten students are required to have an eye exam. Beginning for the 2010-2011 school year, NE state law (See NE Immunization Program) requires a whooping-cough booster shot for all 7th graders. Incoming out of state high school students: Physical waiver forms are available to complete if your student is not planning on participating in an athletic activity. This is a required form to complete for our Health Office.
